Consent and forms

Templates you can hand to a client, or digitise inside Ecko Health and send for a digital signature.

CONSENT

Patient Informed Consent

A one page consent form covering what Ecko Health does, where the information is stored, and the client's right to decline or withdraw. Signature and guardian fields included.

PDF & Word · 1 page · v1.0, August 2026

CONSENT

Telehealth and third party consent addendum

For sessions by video or phone, and for couples, family, interpreters and support people. Consent to transcription and consent to telehealth are separate questions, so this records them separately.

PDF & Word · 2 pages · v1.0, August 2026

SUPERVISION

Supervision consent and record form

A supervisee is not a client, and their record should not look like one. Sets the terms for documented supervision, and what stays out of the written record.

PDF & Word · 2 pages · v1.0, August 2026

DISCLOSURE

AI disclosure notice

Paste ready wording for your own website, privacy policy and intake form, plus the sentence to say out loud in a first session.

PDF & Word · 2 pages · v1.0, August 2026

Clinical guides

Written for practising clinicians. Useful whether or not you use Ecko Health.

MEASURES

Outcome measures quick guide

K10, GAD-7, PHQ-9 and DASS-21. Scoring bands, review cadence, what counts as a meaningful change, and the traps that quietly invalidate a trend line.

PDF & Word · 3 pages · v1.0, August 2026

INTAKE

New client intake question set

The questions worth asking in a first session, so the hour builds a formulation instead of filling a form. Risk asked plainly, woven in rather than bolted on.

PDF & Word · 2 pages · v1.0, August 2026

Practice operations

For the person who ends up responsible for all of this, which is usually the owner.

RECORDS

Record keeping and retention

What to keep, for how long, and who may see it. Fill in your own practice's figures once and put it on the wall.

PDF & Word · 2 pages · v1.0, August 2026

MIGRATION

Practice switching checklist

Changing the system your practice runs on without losing a record, a booking or a client's trust. Includes what to ask a provider before you sign.

PDF & Word · 2 pages · v1.0, August 2026

FOR PATIENTS

Patient explainer

The questions clients actually ask, answered in plain language. Hand it over or email it ahead.

Frequently asked questions

The questions clinicians actually ask us.

Am I allowed to change these documents?

Yes. Change them, put your own letterhead on them, delete what does not apply. Every document here comes as a PDF and an editable Word file for exactly that reason. Your practice remains responsible for meeting its own consent and record keeping obligations, so read what you sign your name to.

Do I need to be an Ecko Health customer to use them?

No. Most of what is here is about good practice rather than about us. The clinical guides and the practice operations documents make no mention of any particular system. The consent documents do, because that is what they are for, and the wording is easy to swap for your own.

Do I have to give you my email to download anything?

No. No sign up, no form, no follow up sequence. Click and the file downloads.

Is any of this legal advice?

No. These are starting points written by clinicians, for clinicians. Consent and record keeping obligations differ by profession, jurisdiction and organisation, and yours may require wording we have not anticipated. Review and adapt before use, and check with your indemnity insurer or professional body if you are unsure.

What happens if a client says no to being transcribed?

You document the session the way you always did, and their care is no different. A client can also decline part way through a session, or agree to transcribing while declining a telehealth connection. The telehealth and third party addendum records each answer separately, rather than collapsing them into one signature.

Can I send the consent form digitally instead of on paper?

Yes. The consent form can be digitised inside Ecko Health and emailed to the client, who fills it in, signs it digitally, and has it saved to their file automatically. The PDF and Word versions here are for practices that would rather do it on paper, or that use something else.

Is there a version of these for supervision?

Yes, the supervision consent and record form on this page. A supervisee is not a client, and their record should not look like one, so the form sets the terms of the arrangement and keeps supervision notes apart from client files.

Something is missing. Can you add it?

Please ask. This page exists because clinicians kept emailing us for the same documents, and the list grows the same way. Email support@eckohealth.ai and tell us what you need.

These templates are provided as a starting point and are not legal advice. Your practice remains responsible for meeting its own professional, jurisdictional and organisational consent and record keeping obligations, so review and adapt the wording before use.
